Approval of Participation in Intercollegiate Sport

Prior to any activity, practice or competition, as a member of a William and Mary Intercollegiate Athletic team, individuals must be medically cleared for participation by the Team Physician or his designee. To initiate this process, entering student-athletes must have a complete Athletic Participation Physical Exam on file in the athletic training facility. Each following year, returning student-athletes must complete and submit to the athletic training staff an Interim Health History Form. Medical clearance will not be granted without complete and accurate submittal of these very important forms. Omitting or providing fraudulent information on either the Athletic Participation Physical Exam or the Interim Health History form may result in dismissal from a team or a cancellation in athletic grant and aid (scholarship).
 
Unless otherwise indicated, the medical clearance process will involve the review of a student-athlete’s medical records and an individual exam by a physician. However, if deemed necessary, a student-athlete may be required to have an individual examination by a SHC physician, medical specialist or the team physician prior to clearance for participation. This required medical evaluation will be arranged by the athletic training staff. The team physician retains the final authorization to approve or disqualify any student-athlete from participation in any intercollegiate athletic activity at The College of William and Mary.
 
If a student-athlete is injured and/or restricted from participation by a physician, the Division of Sports Medicine must have written approval from the attending physician prior to granting clearance for participation in intercollegiate athletic activity.
